Dr. Cindy Jacob and Dr. Shervy Xavier are credentialed USCIS Civil Surgeons authorized to complete Form I-693 immigration medical exams. If you live in Katy and need your immigration physical, TB blood testing (IGRA), or required vaccinations, our clinic handles the whole process on-site. You leave with your sealed envelope ready for USCIS submission, no extra trips required.
Dr. Shervy Xavier is a board-certified family medicine physician who enjoys the variety of care that primary care offers and values developing lasting relationships with her patients. She offers a full range of services, including chronic care management, sports physicals, preventive health visits, women’s health services, and sick visits.
Originally from Illinois, Dr. Xavier earned her undergraduate degree in Biology from Loyola University of Chicago. She completed medical school at the American University of Antigua and her Family Medicine residency at the University at Buffalo. In her free time, she enjoys spending quality time with her family.
