When you receive a notice from (USCIS) requiring a medical examination, the letter will specify that the exam must be completed by an authorized "civil surgeon."

What is a USCIS-Designated Civil Surgeon?

Not every doctor is legally permitted to perform an immigration physical. A USCIS-designated Civil Surgeon is an experienced, licensed physician who has been specifically authorized by the U.S. government to conduct medical examinations for individuals applying for an adjustment of status (a green card) within the United States.

Civil surgeons are trained to ensure that all applicant health evaluations, tuberculosis (TB) testing, and vaccination records strictly align with current U.S. immigration laws and Centers for Disease Control and Prevention (CDC) public health guidelines.

Dr. Jacob

Dr. Cindy Jacob is a board-certified family medicine physician dedicated to providing compassionate, comprehensive care for patients of all ages. Originally from New York and raised in Fort Bend County, Dr. Jacob graduated from Clements High School in Sugar Land before pursuing her medical journey abroad. She earned her medical degree from the University of the West Indies in St. Augustine, Trinidad, after completing her undergraduate studies at Manipal University in Karnataka, India. Dr. Jacob completed her residency at Grand Forks Family Medicine Residency, followed by an obstetric fellowship in Grand Forks, North Dakota, where she received advanced surgical training in deliveries and C-sections. With nearly 15 years of clinical experience in Victoria, Texas, she cared for a diverse patient population—from newborns to seniors—and has performed a range of obstetric procedures. Dr. Jacob has a deep passion for internal medicine, geriatrics, and women’s health. In her new role in Katy, Texas, she is excited to continue serving the community with a special focus on preventive care, physicals, diabetes management, weight loss, and women’s health. She currently lives in Sugar Land with her husband, daughter, and two dogs. Outside the clinic, Dr. Jacob enjoys baking, traveling, and spending quality time with family and friends.

Dr. Shervy Xavier is a board-certified family medicine physician who enjoys the variety of care that primary care offers and values developing lasting relationships with her patients. She offers a full range of services, including chronic care management, sports physicals, preventive health visits, women’s health services, and sick visits.

Originally from Illinois, Dr. Xavier earned her undergraduate degree in Biology from Loyola University of Chicago. She completed medical school at the American University of Antigua and her Family Medicine residency at the University at Buffalo. In her free time, she enjoys spending quality time with her family.

When you book your immigration physical at Primary Care Centers of Texas, you are guaranteed to be seen by an approved immigration doctor who understands the exact CDC guidelines and USCIS requirements needed to prevent application delays.
USCIS strictly requires that all immigration medical exams for green card applicants (Form I-693) be completed by a recognized civil surgeon. While your regular doctor can provide routine healthcare, only a physician officially authorized and designated by the U.S. government has the legal authority to conduct this specific evaluation, order the required testing, and seal your final immigration paperwork.
The primary purpose of the immigration physical is to screen for specific communicable diseases of public health significance, such as tuberculosis or syphilis. If a health issue is identified during your lab work, it does not automatically mean your green card application will be denied. Our civil surgeons will clearly explain the results and provide or prescribe the necessary medical treatment to resolve the issue so you can successfully meet USCIS health requirements.
Standard health insurance plans typically do not cover the base cost of the USCIS immigration physical or the administrative completion of Form I-693. However, if you need specific vaccinations or lab work (such as blood tests) to meet the immigration requirements, your insurance might cover those individual components.
