Who Needs a School Physical?

A routine school physical isn’t just a district requirement; it is a critical milestone for tracking your child’s developmental health. We provide school physicals for students at every stage of their academic journey, including:

  • Early Childhood & Kindergarteners: Children entering a structured school environment for the first time often need a baseline physical to ensure their immunizations are up to date and they are developmentally on track.
  • Middle & High School Students: As children experience rapid growth spurts, an annual physical helps monitor changes in their body, screen for emerging issues like scoliosis, and ensure they are healthy enough for physical education classes.
  • Out-of-State Transfers: Families relocating to Katy or surrounding areas will likely need to provide the local school district with an updated, Texas-compliant physical exam and immunization record before their child can attend classes.
  • College-Bound Young Adults: Many colleges and universities require an entrance physical and updated vaccination records (such as the meningitis vaccine) before a student can live in on-campus dormitories or register for classes.

What to Expect: Our School Physical Process

At Primary Care Centers of Texas, we’ve streamlined the physical exam process so you can get in, get checked, and get back to your day. Whether you walk in or book an appointment, here is what your family can expect during your visit:

  1. Quick Check-In & Paperwork Prep: Upon arriving at our Katy clinic, our front desk team will get you checked in. This is the best time to hand over any specific health appraisal forms provided by your child’s school or district.
  2. Vitals & Medical History: A clinical staff member will record your child’s current vital signs, height, and weight. We will also review their medical history with you, including previous illnesses, surgeries, family health history, and any current medications or allergies.
  3. Comprehensive Physician Exam: Dr. Cindy Jacob or Dr. Shervy Xavier will conduct a thorough physical examination. This includes evaluating the heart, lungs, eyes, ears, nose, and throat, as well as checking joint flexibility, reflexes, and overall physical strength to ensure they are ready for school activities.
  4. Form Completion & Next Steps: Once the exam is complete, your physician will sign off on the required school paperwork. If any immunizations are missing or if a follow-up is recommended for an emerging health issue, we will discuss those next steps with you before you leave.

Every school district has slightly different requirements, though most provide a standardized health appraisal or physical clearance form. Our providers will review and complete this documentation during your visit to verify that your child is in good health. Please bring any required school district forms with you to the appointment.
In most Texas schools, a physical exam is considered valid for 12 months from the date of the appointment. There are occasional exceptions for certain groups—for example, a special education student’s physical may sometimes be valid for up to three years. We recommend checking with your specific school district to confirm their policies before the school year begins.
While they share some similarities, they serve different purposes. A school physical is a broad, comprehensive assessment of your child’s overall health, development, and medical history. A sports physical is a more targeted exam designed specifically to ensure it is safe for your child to participate in strenuous athletic activities. If your child plans to play school sports, please let our staff know Dr. Jacob or Dr. Xavier can easily perform both evaluations and complete all necessary paperwork during a single visit.
If your child is due for any vaccinations required by their school district or college, we can typically administer them during the physical exam. Primary Care Centers of Texas provides full immunization services. To help us ensure your child is up to date and fully compliant with state requirements, please bring a copy of their most recent immunization record to the appointment.
During your first visit, your provider’s main goal is to establish a comprehensive baseline of your overall health. The appointment typically begins with a detailed review of your personal and family medical history, current medications, and any specific health concerns you might have.
