One of the most important documents in your application packet is Form I-693: Report of Immigration Medical Examination and Vaccination Record.
At Primary Care Centers of Texas in Katy, our designated civil surgeons, Dr. Cindy Jacob and Dr. Shervy Xavier, specialize in conducting comprehensive I-693 medical exams. We are here to ensure your form is filled out accurately, sealed properly, and ready for USCIS review.
What is Form I-693?
Form I-693 is the official document used by U.S. Citizenship and Immigration Services (USCIS) to verify that an applicant does not have any health conditions that would make them inadmissible to the United States on public health grounds.
By law, you cannot complete this form yourself, and it cannot be filled out by your standard primary care doctor. It must be completed, signed, and sealed by a government-authorized I-693 civil surgeon.
What Does the I-693 Medical Exam Cover?
Your appointment at our Katy clinic is a comprehensive evaluation designed to complete every section of the I-693 form. The exam strictly follows CDC guidelines and includes:
- Physical Examination: A general review of your physical and mental health history, including a physical exam by our civil surgeon.
- Tuberculosis (TB) Testing: An IGRA blood test is required for all applicants aged two and older to screen for TB.
- Syphilis & Gonorrhea Screening: Required blood and urine tests for applicants aged 15 and older.
- Vaccination Review: A detailed assessment of your immunization history to ensure you meet all current USCIS vaccination requirements.
The Strict "Sealed Envelope" Rule
The most critical step in the I-693 process happens at the very end. Once Dr. Jacob or Dr. Xavier completes your exam and your lab results are returned, they will sign the Form I-693 and place it inside a sealed envelope.
You must not open this envelope under any circumstances.
USCIS will automatically reject your medical exam and return your application if the seal is broken, tampered with, or altered. We will provide you with a separate, unsealed copy of your completed I-693 form for your personal records, ensuring you know exactly what is being submitted to the government.
